Add model card metadata

This PR improves the model card by adding more metadata, such as:
library_name: transformers(indicated by the tokenizer and code snippet)pipeline_tag: image-segmentation- link to the Github repository
It also makes sure to link the model to https://huggingface.co/papers/2311.13385.
